The library stores personal data in accordance with the General Data Protection Regulations (GDPR), the Data Protection Act 2018 and Trust data protection policies.
The information that you provide to us is used to offer you library and knowledge services during your employment or placement with South Warwickshire NHS Foundation Trust. Membership details are entered onto our library management system. Your information will remain on this system for the duration of your library membership. A paper version of this form may additionally be stored to assist the work of library staff. We may keep other paper records relating to specific services, such as training or literature searching.
We will try to keep your details as accurate as possible. For trust staff we will delete your membership and all associated personal details if you have not used the library for five years. Student memberships will be deleted after the end of your course. Memberships of doctors on training rotations will be deleted two years after their last period of employment.
You have the right, at any time, to see what information we hold about you, have any information corrected, request that we delete any information we hold about you. However please be aware that if you ask us to delete some or all of your information, it may affect what services we are able to offer you.
